COVID-19 SAFETY INFO
Hi! Welcome to Five Element Acupuncture with Margaret Hallisey.
I am seeing clients in-person at my 7 Maple Ave. office, in Philmont. I’ve taken a class through the National Certification Commission for Acupuncture and Oriental Medicine(NCCAOM) in reopening safely during Covid-19, and I am following their guidelines.
When you arrive
When you arrive, please text from the parking lot, so you can come straight into my office without sitting in the waiting room. I will take your temperature, and then have you wash your hands, before you come in the room. We both will wear masks the whole time.
If you develop any Covid symptoms prior to your treatment, just call to reschedule. Symptoms include loss of sense of taste or smell, cough, fever, body aches and sore throat.
New: Intuitive Healing Sessions over the Phone
For some people, coming in person right now is not an option. I invite you to consider intuitive healing sessions over the phone.
If you feel more comfortable with phone sessions, I am available for Telehealth sessions. These sessions are an intuitive energetic healing using the principles of Taoism. You lie down, close your eyes, and listen as I guide you through the movement of qi, blood and your emotions in your body, helping things move where ever they may be held. We can also do herbal medicine consultations over the phone.
COVID-19 INFORMED CONSENT TO TREAT
I understand that the novel Coronavirus (COVID-19) has been declared a global pandemic by the World Health Organization (WHO). I further understand that COVID-19 is extremely contagious and may be contracted from various sources. I understand COVID-19 has a long incubation period during which carriers of the virus may not show symptoms and still be contagious.
I understand that I am the decision maker for my health care. Part of this office’s role is to provide me with information to assist me in making informed choices. This process is often referred to as “informed consent” and involves my understanding and agreement regarding recommended care, and the benefits and risks associated with the provision of health care during a pandemic. Given the current limitations of COVID-19 virus testing, I understand determining who is infected with COVID-19 is exceptionally difficult.
To proceed with receiving care, I confirm and understand the following (confirm in all seven places provided)
